Síntese do concurso

I need some graphic design. I would like a business card designed with my logo and phone number 0409062355 on the front. I would like a grey/silver ot silver background with a wallpaper style pattern. Would also like a matching one page treatment menu. I dont have the prices yet so leave blank at this stage. Would like to have my picture on it. I will attach picture soon. I have updated contest price to account for this.

Treatment menu to be 10cm width and 21cm in height.
Content of treatment menu to have logo, picture of me and number up the top.

Lucious lips and fine tuning for lines from $450 per ml
Cheek enhancement from $750 per ml (2ml package $1290)
(The prices may change- I have done this as a guide) also as I go to different clinics I may require the treatment menu to be updated ocassionally by the contest winner .
