I need a document header for a Word document

Contest Brief

The header should incorporate our logo and be approximately 1600 pixels wide by 440 pixels high. We're an Information Technology company, so the design should be fairly simple with few colors that go with our current logo.
We've attached an eps of our logo and a jpg of a current header that we have, but we're looking for something better than that one. We would like the file in a standard image file format--eps, png, or psd preferably.
