Yes, new table for each report and table name is probably most logic to call it the value of "name" like here: "John Doe"

i have a question...do you need to create a new table for each report(if yes what the name of the table should be?)?..or insert to a existing table those values.

the db is a just a new installed xamp with mysql, meaning the script do not have to include the "CREATE DATABASE statement" .
