143968 Web app portal

We are looking to create a portal/web app for managing our customers, allow customer login to view/monitor their server environment, and submit service requests. The user interface must be easy to navigate and have a web 2.0 look/feel. I've included example UI's and request all bids include a portfolio of past work. I'm looking for a modular approach that is flexible and provides for easy feature enhancements and/or additional modules. Please base your bids on modules 1-4 and a second phase to include modules 5-x will be bid down the road.
