Customized Form-to-email form (in ASP)

A Form-to-email like this one: [login to view URL] Please note that for the "Registration Fees" part, when you choose "Member", it will show further information for visitor to choose. Otherwise, it will be hidden. After visitor submit the form, it will jump to a "Confirmation Page", they can click "Edit" button to edit the information again, or click "Confirm" to submit the information. After submit, there are 2 emails will send out. First one, will send to the visitors, give him/her back his/her submited information.
