Ins Call Entry

We are looking for an application with the following fields. MRNo, VisitNo, First Name,Last Name, DOB, Age The Mrno is unique for the patient . The VisitNo is unique for each visit So user should be able to search the patient by mrno,visitno,firstname or lastname and enter a new encounter with VisitNo, admitdate and admittime (defaults to current date and time and should be editable) , Doctor,Unit,Primary Ins, Days Approved, StaffName ,dischargedate When they enter the new visit number for an existing patient, the mrno, patient name, insurance etc should auto populate. But the user should be able to change the insurance details if needed. Before the expected discharge date the staff member calls the insurance company and extends the stay if needed. So each time they call they will enter the call details like Date, INS company called, INS Rep name, INS rep contact number , Reg days approved/denied , Partial pay days approved/denied , Next Review Date, Notes (small notes around 120 letters). If it is a deniel they will enter -ve number. So initially during admission if 10 days were approved and 2nd call they denied 2 days , they enter -2 in call details and the application should subtract 2 from 10 and create the effective approved days to be 8 The staff member can call the insurance company multiple times during the visit. So the user should be able to log multiple insurance calls . We should be able to calculate the approved days (Reg + Partial pay days ) and calculate unapproved days for the stay( Discharge day-Admitdays-regular approved days-partial approved days ). We should be able to print out all patient approved and unapproved days for a time period grouped by insurance There should be separate logins for each staff members(only three members). When the staff member logs in she should be able to select a checkbox to filter and show only her patients who are not discharged . This way she can work from a small list and work with insurance company and update the information. We should be able to run reports to see 1) Total patients group by insurance for a specific date range 2) Patients with visit number ,the review details and totals days stay We should be able to import patient information from a csv file with mr number, visit number, admit date ,discharge date etc. If the visit number already exists for that patient it should update only the discharge date. If not it should create a new entry for that patient visit. We are looking for desktop application done in vb.net or c# with access or sql express back end. We will need the full source code since we will have to make any modification needed internally. We prefer not to use any third party addons. If you are using 3rd party addon we will need a copy of it . I am attaching a copy of an excel sheet that is currently being used .
