I need a skilled medical writer to draft clear, accurate patient-history records for cardiology cases. Each history must read like it belongs in an electronic health record: concise narrative, logical flow, and fully compliant with common clinical documentation standards. These histories will focus on a single specialty—cardiology—and must capture the patient’s current and prior medications and treatments in detail. Surgical background, family cardiac risk, and other elements are not required this time, but if our collaboration goes well I may ask for them later, so structuring the files so they can be expanded is helpful. You will receive anonymised source notes (consult letters, lab summaries, imaging reports); your job is to transform them into a polished, physician-ready history section. Typical length is one to two pages per patient. Accuracy, medical terminology, and consistency with cardiology guidelines are essential. Deliverable: a Microsoft Word or Google Docs file for each assigned patient, formatted for easy copy-and-paste into most EHRs and double-checked for clinical accuracy and spelling.

Clinical documentation for cardiology requires precision in translating complex source material into structured, EHR-ready narratives that maintain both accuracy and brevity. This project demands someone who understands cardiology workflows and can transform consult letters, lab summaries, and imaging reports into physician-standard histories without embellishment or loss of clinical detail. Cardiology Patient Histories Creation demands proficiency in medical terminology, documentation standards, and the ability to extract relevant information while discarding noise. Source material will be anonymized, requiring attention to de-identification protocols while preserving clinical integrity. Each history will be formatted for direct EHR integration, meaning structure, length, and clinical terminology must align with institutional standards. With 6+ years handling medical research, technical documentation, and clinical writing across specialties, this work aligns directly with the requirements. Familiarity with cardiology-specific terminology, medication interactions, and clinical workflow ensures rapid turnaround without revisions for medical accuracy issues. Deliverables will be polished, EHR-formatted Word or Docs files ready for immediate physician review. The modular approach you've outlined for future expansion into family history and surgical background fits a scalable workflow model. Ready to begin immediately with your anonymized source materials.

Hi, I can transform your anonymized cardiology source notes into concise, physician-ready patient-history documentation suitable for EHR use. I’m experienced in medical/technical writing and can structure each history with clear clinical chronology, accurate terminology, and consistent formatting while preserving the source information without introducing unsupported findings. For each patient, I can cover: • Presenting complaint and relevant clinical history • Current and previous medications/treatments with doses, frequency and relevant changes when documented • Cardiology investigations, labs and imaging findings • Relevant clinical course and treatment response • Clear chronological narrative and appropriate medical terminology • Consistent EHR-friendly formatting for easy copy/paste • Careful proofreading for accuracy, spelling and internal consistency I will not infer or fabricate clinical information that is absent from the source notes. Any unclear or conflicting information can be flagged for review. I can deliver each completed history as a Word or Google Docs file, typically 1–2 pages per patient, with a consistent structure that can easily be expanded later for additional history sections. I’m available to start with a sample case and can maintain a reliable workflow for ongoing assignments.

Hello, I’d be interested in supporting your cardiology documentation project. I can transform anonymized consultation notes, lab summaries, and imaging information into clear, concise, well-structured patient histories suitable for EHR use. I’ll pay particular attention to accurate medication and treatment histories, appropriate cardiology terminology, logical clinical flow, spelling, and consistency while preserving the information provided in the source material. I understand that these records must be documentation-ready rather than diagnostic or speculative, and I will not introduce information that is not supported by the source notes. I’m comfortable working with Microsoft Word or Google Docs and can structure the histories so additional sections can be incorporated later. I’m available to start immediately and would be interested in ongoing work if the initial project is successful. Best regards, David

Hi, I'm an MBBS graduate with several years of hands-on clinical experience, currently practicing at a hospital in Pakistan, with a growing focus on medical writing. This project is a strong fit — writing accurate, EHR-ready patient histories is close to what I already do daily in clinical documentation. I can read consult letters, lab summaries, and imaging reports and translate them into a clean, physician-ready narrative.I'll structure each history (chief complaint, HPI, current/prior meds and treatments) so it copy-pastes cleanly into standard EHR templates.I can deliver each patient history as a Word doc, typically 1–2 pages, within 24 to 36 hrs of receiving source notes. Happy to do a sample patient history first at a reduced rate so you can review quality and format before committing to the full batch. Looking forward to working together.
