Application to convert pre-filled PDF documents to HTML

Job Description:
If you have an application or can quickly build one, to convert any PDF document into HTML with generic styling, then I'm interested in looking at your solution. Attaching a sample PDF form.
The input PDF documents will come pre-filled with data. That data needs to be retained as part of the conversion to HTML.
Manual conversion won't work in this situation, because there were thousands of PDF's that needs to be converted to HTML.
Please let me know if this is something you can do..
