I work in a remote hospital in Kenya, and I need to utilize google services(google sheets, forms, docs, etc) to automate data entry, storage, and reporting, or create an app to help do this.(Which ever is easier) This will help us keep track of patients better and allow us to apply for grant funding once we can easily produce records of what we are doing.
I help run a specialty clinic where each patient is seen by a number of clinicians during each visit. I need to allow each clinicians to enter data about a patient. Then, I need to combine the information from each practitioner in one row of a spreadsheet for each patient. I need each row of data to populate a document that can be copied and pasted into an electronic medical record.
I have started trying to automate this process with google forms, google sheets, and google docs. Currently, I have it where each practitioner can enter information on a google form that populates a tab on a google spreadsheet. I have then combined the information from each tab into a master database by using some formulas so that the rows of data are populated based on a key that is made from the patient's unique medical record number and the date of the clinic visit from each practitioner. Basically, I can get the information from each clinician during each clinic into one row for the patient, but I havne't figured out how to make it populate a template that I can copy and paste into the EMR. I would also like some way to record photos on the database if possible.
I have attached a rough draft of what the template would look like and a screen shot of some made up patients on the data base. You can see the basic formula that I used to create the merged information and you can see the tabs that they are coming from at the bottom.
Ability to record information in a clean way(currently using google forms with drop downs)
Store it in a meaningful way (each patient sees 3-6 people during each clinic visit and all information from each provider is stored in one line based on the key from medical number and date)
