Excel to Web Form back to Excel

I am looking for a person to create a program that will allow me to change an excel worksheet to a Web Form that useres can populate with data. The Web Form must allow more than one user to enter data at a time into the Web Form.
Once the Web Form is populated, I want to be able to export the data back into Excel.
The formating of the web form must be the same as the origional excel worksheet. The final excel worksheet must also have the same formatting as the origional excel worksheet.
