Generate Google Map from Zoho Creator Form Data

I need to produce a Google Map from data stored in a Zoho Creator Form, on a zoho creator page. The map must show where goods are sent from and where to. The relationship is 1 to Many. So goods from an address in one country can be sent to addresses in many other countries. There will be overlap. EU (London) to France (Paris), Paris France (Paris) to UK (London).
We should be able to hover over each location and it tells you how many locations this location sends goods to and receives goods from.
Between the 'From' country lines should be drawn to each 'To' Country.
The code can be developed as a page, and all I need to know how the data needs to be presented to the developed script.
