Build a Website, 5 pages, 1 product purchase

Need a quote of time and $ on the following project. A 4 page site. Page 1: Home page, parallax theme (scroll down to see whole content) text and 1 video in the middle of the page, "click here to buy" at the end of the page Page 2 (the "click here to buy page") 3 fields: Filed 1 - Drop Down Select, Field 2 - Upload Resume (file upload- Doc format) Field 3 - Large text input box Page 3 (payment page) your payment method, agree to terms & conditions, Confirm order Page 4 Thank you for the order General idea of the website: Customer open the main page, customer scrolls down the main page and clicks "buy now" and goes to the second page Second page - client uploads a file, and enters text into another field, clicks next for payment Third Page - client enters payment information, (credit/paypal/wire transfer) Fourth page is a confirmation and thank you. Once that is selected, the page takes his file/resume, and sends an email, with the file, and the text in the large text field to the group of emails defined by the drop down. I will need to collect the emails in the database, and have the ability to add email to the groups confirmed by the dropdown. The drop down should have around 10 options. How much would this cost, how long would it take to develop?
