Use Javascript to add Headings to Form - Joomla Component

I have a Joomla component that gathers information for new members. This form is built in Javascript.
I need to add three messages above different areas of the form. Right now they are located at the top of the page, because I was unable to get them inserted into the correct place in the form. Below is where the different messages need to be located.
1. Needs to be placed above the "Type" field:
Renewals
If you are renewing your membership please enter your member number in the "Member Number" field. New members can leave this field blank.
If you do not know your member number, click here to search the member number database.
If you do not find your member number in the database, Email or call 61x-31x-73xx.
2. Needs to be placed above the "Username" field:
For Gift Memberships
If you are purchasing a gift membership, please enter the recipient's name and preferred address (either work or home). Please list the new member's job title, then proceed to the Sponsors and Gift Memberships section.
3. Needs to be placed above the "Do you want to be included on our mailing list?:" field:
Mailing List
Occasionally we make our mailing list available to carefully selected companies. If you do not wish to have your name included, please check "No" for the question "Do you want to be included on our mailing list?" at the bottom of the form.
Also I will need to know which files were changed so that I can go back and edit them later.
