HTML MULTI FORM SCRIPT

To create a login form like that on [url removed, login to view], where based on the item chosen the form is drawn with the following Parameters customizable:

Sort order (REQUIRED)

Username Label (REQUIRED)

Username Field Name (REQUIRED)

Password Label (REQUIRED)

Password Field Name (REQUIRED)

3rd Form Label (OPTIONAL)

3rd Form Field Name (OPTIONAL)

Form Name (REQUIRED)

Action (REQUIRED)

Onsubmit Function (OPTIONAL)

Hidden Fields (OPTIONAL)

Static HTML Coding (OPTIONAL)

Note:

Hidden Fields should be in the format of:

[["hiddenfield1 name","hiddenfield2 value"],["hiddenfield2 name","hiddenfield2 value"],....]

Or something similar so that multiple hidden fields can be entered
