Login Form for mobile application html5 jquery

I have developed an Android mobile app using html5 jquery mobile.
I want a login form that only allowed users can use the app.
The users do not have to register. Best is that the users do not require any internet connection.
I want the app to reset after three days. So after three days, the users cannot login anymore and use the app anymore unless I provide the new password.
