Creating an interactive (dynamic) PDF for a Physiotherapy Assessment

Job Description:
At the moment we are using an normal PDF file that we open on our Samsung tablets. The tablets can be used with a S-Pen. The PDF File is attached. This file we fill out with hand-written notes. Since we are many different people in the company and everyone has a different handwriting sometimes it's hard to read what each employee writes. Now we want to create a interactive PDF with different drop down menus, preselected fields for the date, the text and pictures. The hand-written text should be transformed into a typed text. We are using the app XODO to work on the PDF file. This app transforms the hand-written text.
The PDF file contains different test for different body parts with titles (Shoulder, Hip, Back etc). Each subgroup contains different test. We want to select which of the body parts we want to include in the assessment. If the subgroup (for example "Shoulder") is selected all test for this body parts appear. All other subgroups stays hidden (only the title can be seen).
The attached PDF is not fully finished. It's only so you would have an idea. Long story short. We need an interactive PDF with dropdown menus, text fields, picture fields and selective appearance.
