Hi, I specialize in working with custom-made, interactive PDF forms and I can help you a lot with your issues. I noticed that the file you shared was created in LiveCycle Designer, not Acrobat. Is there a reason for that? Acrobat forms are much more commonly accepted, unlike LCD forms. I can convert your existing forms to Acrobat ones and make sure they work correctly (the price I quoted includes that conversion). I'm available to start working on it right away.
