Needed: an addition of "send email" functionality to an existing simple javascript form.
I have an old form that uses a Perl/CGI script (with NMS [login to view URL]), with simple functionality of sending form output to several email recipients. I created a new form with similar fields using Namo Webeditor, where upon submit the form feeds the output into a database of an email service (Intellicontact). I would like to duplicate the functionality of the old form so that the new form will ALSO email the results, while still feeding them to the database. I do not care whether that is achieved using the old Perl/CGI program or simply done in JavaScript (I would prefer that latter).
The field verification function is already done in the new form, and that functionality does not need to be duplicated from the old form.
Plus small additions to form:
* 2-3 simple if /then JavaScript functions added to generate hidden fields values (example: user indicates "state", that determines a hidden field called "region"; I will provide the simple rules).
* generation of a date /time stamp field for submission date
I know this is all very basic... less than hour work for someone who knows what he is doing

Quick turn around appreciated
All code must be fully commented
I expect to be able to fully test the form before payment.
I have several similar jobs in the queue.
see attached files for links to the existing and new forms, and the old CGI.
If sending email and storing the form data in the database is all you want, i can do it using javascript and php. javascript for validation which you already seem to have done and php for backend functions. Lets start :-).
