During the visit you will: • Capture 7–8 clear, geo-tagged photos with a GPS-enabled map camera. I specifically need the building exterior, department interior, signage showing the institute’s name, an outside view that includes the signage, and internal shots such as notice boards or any letter-headed material on display. • Present a short form to the issuing authority and fill it in with the required details. • Complete a separate site-visit form with your own observations. • Request the Business Registration Certificate; either photograph it or note the reason it cannot be supplied. • Ask for the hospital’s total bed capacity and record the figure or the reason it is withheld.
