I'd like to make this 2 separate links
the revised sentence would look like "Request Information / Email School"
"Request Information" is 1 link
"Email School" is another link
"Request Information" leads to a page like this [url removed, login to view]
The new title would be "Request Informational Materials"
Instead of the text that says "Fill-out the..." it would read: "Use this form to let [insert school name] know that you are interested in learning more about their school. Provide your information below to request informational materials such as school brochures and catalogs.
"Request from: [insert school name] (in same style-font as this page: [url removed, login to view])
On the bottom of this page, instead of check boxes for the newsletter or get contact by consultants, have 1 checkbox for "Share my profile so that other schools can contact me and send me informational materials." (make default not checked)
When the user clicks "send" a plain text email is sent to the admissions office of that school with the relevant information
Plain text email must be well-organized
Disclaimer at bottom of email should be same as disclaimer on other admissions office emails we are sending
If the user checks the box for "Share my profile...," the profile is also saved to the prospective student database, otherwise, only an email is sent to the school
The thank you page should be the same as the contact schools thank you page

Insead of the line (above the message box) "...please include your mailing address if you are requesting information" write "if you are requesting informational materials, please use this form" (link to "Request Information" page)
Thank you page is the same. Except, add a line that says "If you would like to request school informational materials such as school brochures and catalogs, please use our Request Information form. (link "Request Information" to that page)"
Tracking for emails sent remains the same
Create a new tracking mechanism for the "Request Information" messages and add it to the the school stats page
We'll need to test new pages / functionality on a dummy profile before going live
