1. User enters username and password to enter website. The user has the lost/forgot password option. User has the option of requesting a username and password be creted for him/her.

3. View Patients Screen:
This page starts with two options: New Patient, Active Patients and In-Active Patients.
Active Patients Screen
Table Format: List of Patients Name, Counselors Name, Case Number, Entrance Date as well as a column that tells the user whether or not this “patient file” has documents that have not yet been approved by management. You should be able to sort by any of these attributes. Once you click on the patients name you will go into the next screen.
Patient Detail Screen
The top half of this screen will have the patients headshot photo in the upper left hand corner of the screen and the contents of the patients table spread across the top.
The bottom half of this screen will offer the ability to perform the following tasks:
I. Update Patients Profile
II. Update/Create a Treatment Plan – Make this the last thing you do. Let me know before you start constructing this section.
III. Update/Create Progress Notes – Make this the last thing you do. Let me know before you start constructing this section.
IV. Update/Create Group Notes – Make this the last thing you do. Let me know before you start constructing this section.
A. Patients Profile – Update/Create profile information such as name, address, phone number, birth date, person to contact in case of emergency etc . . .
B. Treatment Plan – Each patient will have ONE treatment plan during his/her stay. This form accepts data from the user. The data is sent to the database. The form remains on the screen. After editing is complete, and the data populating the form is residing in a database, the record has a flag that denotes the record as awaiting Management Approval. The form goes through three stages: Editing, Awaiting Approval and Approved. Upon submission of the data by the user, the data must be formatted on a pdf form EXACTLY the same as “Exhibit A”. This form must be stored and submitted by the hospital in this Approved Format.
This is the most important form in the application. Lets take a close look at the objectives of “The Treatment Plan.”

Each Problem Area has the following “Attributes”:
I. Date problem identified.
II. Statement of Problem.
III. Statement Of Goal.
IV. Short Term (checkbox)
V. Long Term (checkbox)

VII. Target Date – This date should default to 90 days from the “Date problem identified” date IF “Short Term” is selected. IF “Long Term” is selected then the Target Date should default to 145 days from the “Date problem identified” date.
The user will have the ability to preview, print and/or store the document on the local computer.
The number of characters in each section should be limited as not to change the change the formatting of the output page. This data is populated by the user but approved by the manager. Once the manager has approved this form, it can no longer be changed by the user.
C. Progress Notes – Each patient will have many Progress Notes during his/her stay. Use “EXHIBIT C” as a guide. As long as the information appears on one page in pdf form, this will suffice.
D. Group Notes – Each patient will have many Group Notes during his/her stay. Upon submission of the data from the user, the data must be formatted on a pdf form EXACTLY the same as “Exhibit B”. This form must be stored and submitted by the hospital in this Approved Format. This data is created by the user but approved by the manager. Once the manager has approved this form, it can longer be changed by the user.
The user will have the ability to preview, print or store the document on the local computer.
