This project entails taking some quizzes that we use for customer evaluations and making them somewhat dynamic. The project entails a small .NET solution (I am open to other languages as well, however prefer .NET such as c# or vb backends), that has several grids-like pages on it (see attachment, I know the pictures arent really good, they were drawn really quick, however the general idea is to take these grids, and make it so that people can check mark the different areas). There are 7 pages, and I have already constructed the design in a .PSD file. The coder will need to take this .PSD file, and convert the quizes (which I will type up the text for the widding bidder) so that a user can select an option out of the grid.
The grids should only allow one logical choice as well. There is one quiz that we need to make not using the grids, but like a bar that a person can mark or pull up to rank (see the 4th quiz), however I am open to other suggestions on how to approach this.
Also, there needs to be a page written up that can sum up the results of the quiz. Graphs of the information would be amazing, however computation of the entries and display in numerical output is ok as well. Graphs (can be simple bar graphs etc.) would be a really, really desired option however.
The data will also need to be dumped into our SQL database, and have an option when the users are done taking the quiz to send it to a printer to be examined by a customer representative. There will also have to be a simple login in of course that takes the customer name etc. and dumps it into an SQL database.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
