Outlook plugin sample

I need a plugin for Microsoft Outlook (2000 or greater) which puts two buttons on the Toolbar, one that will be considered 'spam', one that will be consider 'not spam'. The button should display an icon. When the button is pressed, it should open up a file dialog (nothing special, just GetOpenFileName()), and save both the header and contents (unmodified) of the file into a textfile. The 'spam' button should also perform one more step, moving the message in question into a folder in Outlook called 'Spam'; if the folder doesn't exist, it should be created. All messages processed by the plugin should add a test header to the message, as well as a footer on the end of the message, which should be visible inside of Outlook. There doesn't need to be any additional logic inside the program (it doesn't need to identify spam), just illustrate how to hook into Outlook.

The program must be written in pure C, not C++ or using MFC/ATL. It should compile on the latest version of MSVC (v7).
