We would like a form built that will feed into a database, that can eventually feed (or export) into an excel document. We need the code written and fully ready to be integrated to the site. This form will be used on a book website that will collect information from people who want a signed bookplate. We will need to collect name, address, city, state, zip, country, would you like to be added to the email list check box, and a notes field for details on how the reader would like their bookplate signed. Our hosting service has SQL databases available. We have something similar setup now, but we get a lot of spam entries, so steps will need to be taken to verify the correct information is being entered. No web addresses in the email fields, and check for common spam words. I will attach a recent spreadsheet that was generated with copious amounts of spam. We have to eliminate the spam problem. We need a private interface built that will give the user the option to download the most recent excel spreadsheet, gives the date of the most recent download and gives the option to clear the spreadsheet.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
