oscommerce - additional fields in checkout process

The new fields must be stored in the DB as well as captured with the order and included in the email to the store when an order is placed. All additional fields can be null. In checkout_shipping I need (Under the shipping address box but above the additional comments box): -header to be in bold (text: "If you ordered a duplicate, please fill out the information below:") -Are you taking a NYC exam? If so, please specify title and number of the exam. (input: one line of text) -What type of duplicate are you requesting: (input: checkbox values: evaluation; translation) -If you are requesting an evaluation, is it a: (input: checkbox, values: diploma evaluation; course-by-course) -Approximate date of initial evaluation/translation (input: line of text) -File Name (if applicable): (input: line of text) [url removed, login to view] To be inserted under "Payment Method" box. heading: Additional Information Needed (Only if you ordered a duplicate) -Social Security Number: (input: line of text) -Social Security Number: (input: line of text).
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
