(I've had two different coders fail to deliver on this project; please do not bid unless you are confident that you'll be able to complete this project. thanks!) I need a PHP price request calculator, similar to the one at:

This should have a secure administrative back-end section, where prices can be modified. Furthermore, it should have a built-in PayPal module where customers can pay for their order (either using their PayPal account or by credit card) which should also include a complete tax invoice at order confirmation, showing:
* the sticker ordered
* the customer's name and address
* my name and address
* the price, inclusive and exclusive of tax and including shipping[1]
I would like to be able to specify a number of different shipping prices, depending on how many stickers the customer has ordered. ie. if they order 1,000 stickers, $30 will be added to the invoice for shipping but if they order 10,000 stickers, $280 will be added to the invoice.
This should then automatically send the customer a thankyou/confirmation email and then send me an email with the details of the payment and the order (although preumably PayPal will take care of this side of things?)
I have enclosed a mock-up of how this should look. All HTML markup should be CSS standards compliant.
[1] It should automatically itemise the various costs, i.e. stickers cost $400 then it should read:
Sub-Total: $330
GST: $33
Shipping: $37
Total: $400
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
