I need a very simple document management system that allows for the following: 1- Web based, written in PHP-5 with MySQL-5 backend 2- Customer Information a. Must allow for add/edit/delete of a customer b. Customer profile contains mailing address, email, phone 3- Document Management (viewed by Customer) a. Documents are uploaded via web browser and "assigned" on a per-customer basis. b. Associated with each document is doc name, title, and notes c. Must save above information along with date uploaded. d. If the same file name is uploaded for a customer, create a new "version" -- e.g. [url removed, login to view] -> [url removed, login to view], [url removed, login to view], etc. 4- Viewing Capability a. By Customer - click on customer name (Business - Full Name) then displays files associated with that customer. 5- Login a. Must have a username / password to login, preferable to use PHP Sessions. Username/password to be stored in MySQL database. 6- Other Requirements a. Must run on BOTH Linux and Windows. b. Must include a configuration file that allows me to easily change the DBname, DBuser, DBpass and file storage location.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
