web based form entry with a searchable database

I am looking for a web based system with the following modules: data entry: a web based form where a user can enter information and save it in the dataabse. Search Engine: A user should be able to search the database using keywords or more thru pull down menus and the results need to be displayed ina specific format. Dashboard: A graphical view of the database contents to be displayed as Pie charts and or bar charts. In this view, a user can click on any of the segments of the pie or on a bar in the bar chart to retrieve relevant information from the database and present again as a pie chart/bar chart or a table i,e text. Normal usage of the system is thru the dashboard module. Users can choose to enter/edit information by clicking on an edit button. At present my database consists of 25 tables with 12 entries and I expect it to grow to about 500. I am open to suggestions on the implementation. I will provide detailed specifications for the database and the webpages/forms. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform AJAX, Linux, MYSQL, JSP
