Modify upload images system

I am building a webpage [url removed, login to view] and i need someone to finish some small tasks its mainly design. (if solved right I have small tasks )...
This project is about uploading images to my website that are property images. I want it to look more or less like this: [url removed, login to view]
scroll down to upload images. My system is capable of uploading images but one at the time and its standard uploading of one image file. I want 5 images in similar design as in link. With no comments in images.
The site is made with zend framework and someone with experience in this is essential.
