Create a customer file and another specific query

Hi Freelancer.. Iam in search for an PHP and MYSQL Professional. My Task is a very easy one - so i also allow Students or any who is able to get the job done. Iam searching for someone who would create me a query for my Website. Smething like "First, Last Name, E-Mail Adress and at least Birthdate" Birthdate should be set to be at least 18 years old. (German Right to be adult) This query should be putten into my Serversided Database (MySQL, Myphpadmin tool) After that query - i need to let the User get an E-mail to confirm that. By Code or SessionID.. Code (arbitrarily) would be the best way. after sending the first personal Information - should stand there "Please Confirm you E-Mail" and on German Language it would be "Bitte bestätigen Sie Ihre E-Mail" After thats done (entering Code) it should redirect to the 2nd Step. - Registering Site Link / Image / Flash should appear with some words " - Like : Register here to participate in the raffle or on German: "Registrieren Sie sich hier um am Gewinnspiel teilzunehmen". Under this Refferal Link / Image / Flash - should be an Textfield: Where the Registered Users should could enter 3 Words out of Registration Affirmation Mail that should be checked - to get to Step 3. On Step 3 there should only be an Text like: Dear Mr. / Miss Bla Bla (out of first query) thanks for your participation - Winners will get Informed my Mail. or on German: "Sehr geehrter Herr / Frau Bla Bla. Vielen Dank für Ihre teilnahme - Gewinner werden per E-Mail benachrichtigt" to then have the "Back to Mainsite" Button appear. (German: Zurück zur Hauptseite) Thats all.... The only condition is - it must be centered in a Surface of width: 550px; height: 380px; with margin : 0 at top and left. - iam going to show that in an <div> - also needed is an css style script. to let the page look like my own. -> [login to view URL] Also wanted is an Admin "Area" - to extract the Mail Adresses from Database - and a kind of "Counter" who really got up to Step 3.
