PHP Form Need Fix

I have a PHP form set up that's purpose when someone fills out the form is to direct them to a thank you page then the information is emailed to me. There is a glitch in the form right now so when the user hits submit the form is not submitting properly. An email comes through but without the data. Thank you for your bids. PLEASE NOTE: I would like to start on the project right away as this is a quick fix so if you cannot start right away and finished today please do not bid on the project.
