Survey Development [WordPress]

We are looking for a coder to create a AJAX survey that will work within a WordPress page.

The survey and it's scoring algorithm are attached titled "Berlin Questionnaire.pdf".

The servery will identify the participant's risk group at which point if they are at risk we will request additional information (to be determined, a minimal list of questions so we can contact them).

The results of their survey will be e-mailed to a designated address with the calculated results.

Ideally I would like a auto-responder in place and for a interface within WordPress itself however a dump of all survey information to a csv/xls file on the server will suffice and I can program outlook to do auto-respond.

In the future we would like to automate the delivery address based on the physical location of the participant based on IP address localization. This feature should be quoted additionally in the comment aside from the main project if possible.

The presentation of the survey must be clean and neat with auto expanding sections if necessary.
