PHP Mail script with upload image as attachment

Hi, I want a complete working script, that has the html form and the php script that handles the sending (it would probably have MIME). The Html form should have the following: (Name Field, Phone number, City, email, Comments textarea, and the most important thing Browse button to upload a picture (Gif, jpeg, bmp), and this pic should be included in the email as an attachment. The form should have javascript validation, and the user should be able to choose to whom the email should go using a drop down menu with two options (i.e. two emails). I want the complete script to be workable. Before I pay you, I will download and install the script on my server, and if doesn't work smoothly without any configration (except for changing from, to, subject), I won't pay anything. This should be a really simple project, or you might already have the script ready, I just don't have the time to look for a script on the net. My budget for this project is between $5-$10 Thanks.
