PHP Cart Modification Needed - Real Easy

This should be really easy. We need to add to the cart somewhere in the checkout process some coding to identify/look for one type of product and be able to offer one an attribute and prompt the user with a drop down for it. But only offer that attribute one time - at checkout. Example Prompt: 'We noticed that you ordered a shirt, would you like 2nd day shipping on this ?' And then have a drop down (YES (+40) , NO). We would need that placed somewhere in checkout. *** This attribute will only be added to the total price, you will NOT have to do any coding for shipping Mods, etc. Should be really simple. Looking to have this done today. Also, it's OSC , so that should make it easier.
