Make a simple webpage from a already made user management library

Firstly, I will only accept jobs that are into budget, because i'm requesting a simple job, for an application that meet the requirements but will not be used for a production environment. Requirements: -User System (Consider use the already made library UserCake, links below) -User Registration Form: it must have the following fields: -RUT: it's the national identity number (example of RUT: 17.522.144-1 , it contain dots and slash). No validation required. *IMPORTANT: this will be used as username to log in* -First Name -Last Name -Birth Date -E-mail -Medical prescription: here the user should be able to choose different medications, that will be stored on a mysql table. User can choose more than one medication, so it should be made with check boxes for each medication -Date of medical doctor's visit (should pop a calendar) -Medic's Name: a drop-down list with all the Names stored in table 'doctores' (tables explained below) -Password -Repeat Password -Security Code (captcha, optional) Screenshot of how it should be (you can use an already made design): Register Page: [login to view URL] -Logged User Main Page: when an user logs in it has to show a webpage where the user can see the following information: -First Name and Last Name -Birth Date -Medic's Name -Date of the doctor's visit -The medicines that were chosen at register time. It must show name, description and url There should be a mysql table 'medicamentos', with the attributes ID (PK), name, description, url (able to put an URL). There should be a mysql table 'doctores', with the attributes ID (PK), Name, job, phone, email. The data of that tables will be manually added to the mysql database, so no administrator page to add fields to 'medicamentos' and 'doctores' will be needed You have to use the following script to get the website to work: [login to view URL] - a PHP user management system, so you have to modify to make the changes like using RUT to login. With Usercake the following problems are solved easily: -Manage Log In -Manage Register (You will have to modify the already made register form to meet the requirements) -Make an Page available only to logged users (You will have to modify the page to meet the requirements) Important: we will accept only projects on budget Important: we will accept only projects on budget Important: we will accept only projects on budget If you accept this job, you will have to: -Use your own developing environment (XAMPP for example) -Notify when you get to the following milestones: -Installed Usercake and have it working -Modified Register form to include the fields and calendar -Modified the page that only logged users can enter -Send screenshots when you have something to show -Send the application (full working directory and .sql file) once it's done. Before you accept this job you will: -Be asked how you want to be paid This is a serious post for a very simple application, but keep in mind that I have other complex projects in progress, so If you do this project successfully I'll be looking for others higher budget and complex project.
