Web Application for Desktop

We are building a custom management application for a client. We will develop the design and layout for this but need an expert application developer. This is a web based application that will store information about users. This is very detail heavy, so to sort out people who do not pay attention to detail, please start your proposal with the phrase UK1 - if we don't see this in the beginning, then the proposal will not be considered. Basic functionality is as follows (see attachment for more in-depth) - Admin has their own login (customers will never login, this application is for keeping track of the customers -- sort of like a customer relationship management program) - Admin registers customers with detailed profile information - Admin has 5 separate events that happen to a customer (they call the customer 5 different times) with 5 different sets of questions. Each question needs to be responded to and stored in the database. - Admin needs to be able to run reports on all questions (can choose from any of the 5 sheets of questions). *1 sheet of questions is roughly 10-15 questions that need to be logged See attachment file which contains the skeleton of what this project looks like along with a wireframe on the last page.
