71004 Online application to PDF file

I need an online application that will take the answers submitted to populate pdf files that are then printable by end user. The data from the application also needs to be added to a database that has an admin section. I have FDF merge that will make this task easy, I just need someone to create the online application. There is logic involved in that if answer is A then B. I have this logic and will share it with those that can provide similar applications and show ability to create such a form. Anyone bidding on this project must have a portfolio of similar work and be able to provide a sample. Generic bids will be ignored so please include my name (Darren) in your bid.
