I am looking for someone to revise an existing form that I use in my business.
It is a one page mortgage lead sheet that I use to collect a borrower's information for a loan application.?
I currently have a sheet that I use, but it's clumsy & needs revisions.
I am attaching a copy of the existing sheet, **but I don't want that to influence the design of the new sheet** - It's for reference only...**i'm not happy with the look of the existing sheet!!**
I'm thinking of keeping the format as a Word document, but am open to other suggestions.
One of the ideas I have, but am not sure if it's doable is as follows: We need to collect a 2yr employment & housing history on each applicant...many times one set of fields is enough, but sometimes it takes two or three sets. It would be nice to display just one set of fields, with a checkbox, etc if additional fields are necessary & then the 2nd, 3rd set would appear. Not crucial, just an idea that would make this form easier to use.
I am hoping for this type of input as to how to make this sheet a more streamlined, better form.?
The fields are mostly stayting the same, with some fields being removed, and several others added.
