I need a website built for a transportation company's clients (Insurance Company or Hospital) to enter trip requests for their patients. The user will need to be view trip orders and make changes to future requests and manage address for their patients (active or inactive)
I need a website that has special registration and permission levels and can that can use google maps to confirm and geocode addresses.
Registration;
We will supply a domain names of users that will be allowed to register.
The user name will be the email address of the user.
A new user will register with their email address and the site will use a stored procedure to call the database to verify that the domain on the email is a valid domain. If it is valid, the user will be sent an email with a link to continue the registration process where they will finish setting up their profile.

There will be an administration page where we can view the users and assign them to a group. Later when a user places an order they will be able to see the history of orders made by them or anyone else in their group.
Our order section will have several parts but the critical piece for this project is a pickup and drop off address. The developer must store a properly formatted pickup and drop off address along with the lat/long for each address. For the purpose of this project you can download a free developer key from google maps. The address interface must be easy to use and present the user with a properly formatted address for acceptance and a map image to verify the location. If the address does not verify then the user must be presented with the ability to reenter the address or tell the system that the address is correct and then require the user to enter cross streets for the address with directions from the cross street.
Once an order has been finalized the user will be able to save the order and a confirmation email with the order details must be sent to the user.
One more thing, all the calls to the database must be using stored procedures to read and write.
Thank you for considering this project.

Thanks for your questions. I am attempting to answer many through this broadcast. I am attaching the PDF form we currently use to collect order information. You will notice there can be three people added to an order. The first person on the order would be the primary person the address would be linked to. When entering trips for a person, if the person has a previous order, the address should have a drop down with the past addresses and when and address is selected from a previous order, the other fields for that address are filled in based on that previous order. For example the contact name and phone number comments etc... Also, there should be a drop down for the user to see previous names so they can just select the name and have the rest auto fill. There is no online billing component as we bill on a monthly basis to the insurance company. The reason for using stored procedures is for security. no direct queries against the database except through the calling of stored procedures. I hope this helps and thanks again for your bids.
