Marketing Questions / Offers System Front end and Back End PHP(repost)

**Overview:** We need to create a system in which we can create questions that a user is given the opportunity to answer on our website so we capture the lead. The system will store all the questions and we will be able to select which question(s) to appear and in what order. A user is given a yes / no choice to answer the question we ask. If they choose the YES radio button a drop group of fields will appear to collect their information. The questions on the page can be limitless, it all depends on how we choose what to display and how many.
