The workflow is that a user would complete the Self Assessment form and it would populate a gantt chart with tasks, timelines and resources that could be edited. A ability to upload documents within the form and and be viewable in a document library that can be search by principle and tags.
A copy of the form would need to be saved and keep previous versions and tasks would be viewable from the project management element.

Create a assessment form based on the PDF Document of questions. The form is actually 76 pages
Have the ability to add resources, actions and dates to each question area
Show a list of actions with resources and dates
Show actions in a Gantt Chart view
Provide a % complete total assessment and per major section and sub section
Separate login account that can only view their own data
Forgot password option
A document library which can be viewed in the form and via a document library.
Add any form to the "project management" engine
