Registration form - wordpress plugin

Hi,
I am looking for a developer of a wordpress plugin of a website registration form with mobile responsiveness and functionality as specified below.
After the registration, the plugin must:
1) Send an email with confirmation of the registration;
3) Send 1 email to each user at random frequency, minimum frequency - once a month;
4) Send the final email after X number of days since the registration.
Please note:
1) Emails should have a customizable HTML format to modify the template based on need.
2) Form field names must be easily modifiable to enable easy translation.
