I need you to create a [url removed, login to view] (and a config file that contains variables like the MySQL database username, password, etc) using the attached files as a template, that does the following: On [url removed, login to view], the form needs to capture 5 pieces of data in this order: Make - a drop-down list containing all the major brands of cars/trucks/etc - we need to find a drop-down list somewhere online of all the major car brands and just copy that to our form. I can help find it if you have problems. Type - drop-down - the choices for this are: Sedan, Convertible, Van, Sports Car, SUV, and Hybrid. Name - this is the user's first and last name. E-mail address - reconfirm their email address from the last step (I'm making them enter it again because there is no easy way to carry over the data from when they entered it on the home page) Zipcode - reconfirm their zip code (for same reasons as above) After entering this, they click "Continue" (I'll provide graphic) Then we need to write all of this information into a MySQL database (a unique auto_increment ID, Make, Type, Name, e-mail and zipcode) After the data is submitted to the database, it needs to send them to [url removed, login to view] and pass the variables of Make, Name (first name only), and zipcode so the new page can make use of them in text. (If you could do a quick lookup on the zipcode and pass the city to the next page instead, that would be preferred) Other than that, [url removed, login to view] is just a blank page using the same attached template that I can fill in with text and make use of the variables. I have already created the MySQL database on my server and will give you the info when I accept your bid. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
