Errors on php script need fixing asap

Our php script is producing some errors when the script is calling sql database for information. This script is a custom ad network. When the ads are called, these errors sometimes appear in the ads or in place of ads. I need this fixed asap so it want jeopardize the quality of our network.
I think second error occurs, based on a suggestion, because the database connections are not closing. We posted this project earlier but couldn't find a programmer we were comfortable with.

Conclusion:
We are seeking someone who can quickly analyze this code and make corrections that will be permanent and not temporary fix to stop errors from occurring.
