Need to import from glucose testing devices.

"Project D" is a software package for Diabetic people. I'm about halfway through "Project D", and I'm looking for a feature that imports data from a blood glucose testing device. The following is a competitors screen shot of this utility: <[url removed, login to view]> It's extremely ugly, but functional - the most important thing. I will handle the GUI in the end. Basically, don't worry about making it look pretty. What I need from you is a form that allows a user to select a testing device from a dropdown listbox, a dropdown listbox for the com port, and possibly a few command buttons (import data, cancel, exit, etc).

1) Complete and fully-functional working program in executable form as well as complete source code of all work done.

2) Exclusive and complete copyrights to all work purchased.

Windows 2000 and Windows XP.

VB .NET 2002 or 2003.
