I need a script that will allow people to create surveys – much simillar to those at [url removed, login to view]
Here are the features:

random question order for each user
I need the user to be able to put his survey into email and it should work. I also need him to be ble to have people click a link to get to the survey.
I would like to have a progress bar for people filling out the survey to know how many questions they have left.
I need the owner to be able to create skip logic.

I need the look of the page where the survey will occur to be customizable so that a person can make it look like the rest of his webiste.
I want it possible for the administrator to be able to give people rewards through download links for answering specific questions
I want the administrator to be able to create a custom page for each person filling out the survey based on their answer.
The person fills out a survey. In that survey, they say that they are 35 and have a big family. They also say they are unemployed. After the person goes through the entire survey, they are then taken to the custom page that has a specific message based on their answers:
for instance: „As a 35 years old person with big family, you care about safety, but because you're unemployed, you are not sure how you're going to be able to provide for those you love”

I want people to be able to access their custom page after they signup with their name and email through a name capture form (the capture form will be generated by Aweber, GetResponse or Infusion Soft.- according to admins choosing)
I want the user to be able to turn off the optin feature (above) if he desires to do so. He'll then be able to show his users the custom page without them opting in.
The Survey will be able to be pasted into a blog if needed (and it will retain all functionality)
The script will need to be installed on a domain.
I obviously need a reporting system to go with this. The administrator needs to be able to see which answers have been put in.
