**Web database ASP and PHP**
1Database with datantry-page published on website that produce a unique case number for every dataentry page that opens. The case number shows in the heading of the mailform.
2 The dataentrypage consists of
-a fixed introduction text
-name and address fields
-tick off box of whether user want to be anononymous
-if anonymous, name and adress fields closes
-tick off whether user is employee of company or institution
-then two fields which can be filled out by users:field for heading, and 6 line textfield that can expand.
-last field: Message field for messages from administrator to user next time they log on
-must have option for attaching files
3 When dataentrypage is filled out and Send button clicked a email is sent to administrator.
4 Administrative panel, password-protected, for easy change of introductory text, number of fields, adressees for the mailform, size of fields and passwords. One field called messages is available for posting messages between administrator and users.
Administrative panel has access to each record entered into database by users. Administrator can post messages in messagefield to users via the panel. These messages can be read by users logging in to database with casenumber as password.
5 Database has two entrances/links for users; one for returning users who will use the casenumber received when they entered data as password. And one for new users, who do not have a password, but are transferred directly to dataentrypoage.
6 Administrator receives email notification when new text is entered into this page or new message is posted by user.
7 Each dataentry has to be treated confidential. User tracking must be blocked on the dataentrypage.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
