HTML Tables Template

I need an experienced HTML designer to create a web page template that can be modified by myself.
It has to be created using tables, no CSS please. I've attached a graphic (.jpg) for you to see what it sould look like (exactly), once the design work goes on.
key requirements:
1. header table (with comments of where it starts and ends), the header must contain 2 columns, as you can see, there is a basket on the right.
2. main body table (where I need to be able to add more tables and cells - IMPORTANT. The body is the 'black part, with a white glow on it', this needs to be expandable in length/height, i would like to have clear comments on where it starts and ends.
3. a footer table at the bottom.
Ideally, I would like four files.
1. a sample template with all files together
2. a separate [url removed, login to view] filie
3. a separate [url removed, login to view] file and
4. a separate [url removed, login to view] file
please do not send any dreamweaver files, where it simply ends each file with '</body> </html> this should only be on the [url removed, login to view] as we're planning to build separate components on this site.
Your quote must be nearer the $30 as this should be a 10 minute job for the experienced.
