joomla 1.0 service order component

We need a component designed (or adapted) which will: 1. allow guests to fill out a form requesting a service (see attachment). This form can be combined with the registration so that a user account is created at the same time. The form results are emailed as a formatted PDF file to the admin, and a database is updated. 2. Users can log back in at any time and check the status of their work order. 3. Admins can access the customer service requests through the backend and add notes documenting the progress of the customer’s work order. This can be as simple as a textarea that the admin updates each time when he wishes to inform about the service order progress. 4. Customers can log in through the front-end and be able to view the admins updates and see how their work order is progressing. 5. Admins can have flexible access to the database as needed, to view reports, etc. 6. Ideally there would be some way to notify customers when updates have been made to their service order. In some ways this is similar to a helpdesk component, but it needs to be adapted to my client’s industry and allow for easy customization since we need to capture many fields of data. It can also be seen as similar to the rent-a-coder interface, but could be much simpler. Please either ask questions or give an overview of how you would create this component in your bid request.
