Our company offers a single product known as MEDSHIELD... It is a Medical Expense Supplemental Benefit that is solicited by Group Health Insurance Agents/Brokers (Producers) to employers who offer their employees group health insurance.
With our key employee no longer with company, we need programing for revisions/enhancing/future updates to our current EXCEL Rate Calculator ***Includes:** Various Rates / Primary Plan Data / Proposal format / Deductible charts / Group census / Claims Example.
Currently there are two calculators, one for our Producers with limited access and one for our UA Officers with full access. We would like to have one calculator with passcode to get full access assigned by UAA as needed.
Hard copy data (RATES) consist of 10-12 pages of a couple hundred possible different rates based on age(s) of employees & benefits chosen. Our policy is increasing our benefit options and we will need to adjust rates accordingly.
The link below is an example of current Rate Calculator. No one knows the formulas or passcodes to allow access to revise -- it is critical that we either create a new Rate Calculator or break into this one and enhance & revise.

**Completion of project needed before end of June.** Will directly correspond with winning bidder all information available regarding the MEDSHIELD RATE CALCULATOR to move forward with this project once bid is accepted.
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
